A standard three initial monogram is where the first name initial is on the left, the last name initial is larger and is in the middle, the middle name initial is on the right.
Example:
name: Jane W. Smith
monogram: JSW
How do you monogram for boys? TOP
Boys monogram is often done with regular initials.
Example:
name: John E. Smith
monogram: JES
